In order to practice and play lacrosse at Riverside HS every student needs to turn in four forms.

The OHSAA Physical, Emergency Medical Form, Sudden Cardiac Arrest Form and the Concussion form.

The forms are available on the Riverside disctrict's web site in the Athletic Department section.

Fees and forms should be turned in to the school's office.
